COMBINATION SHORT FORM OF STRAIGHT BILL OF LADING ISSUED AT SHIPPER’S REQUEST.
FORMULE COMBINÉE ET ABRÉGÉE DE CONNAISSEMENT NOMINATIF ÉMIS À LA DEMANDE DE L’EXPÉDITEUR
- of the Railway Traffic Liability Regulations, SOR/91-488, August 14, 1991 when said goods are carried by a rail carrier; or
- of the bill of lading of the water carrier as provided in its tariffs of Rules and Regulations when said goods are carried by a water carrier; or
- of the bill of lading set forth in or prescribed by the relevant statutes and regulations pertaining to motor carrier services including the Uniform Conditions of Carriage when said goods are carried by a motor carrier; or
- of the bill of lading set forth in the Quebec Regulation respecting the requirements for bills of lading, O.C. 1198-99 of November 3, 1999, when said goods originating in the Province of Quebec are to be carried by a motor carrier;
and which are agreed to by the shipper and accepted for himself and his assigns.
- du Règlement sur la responsabilité à l’égard du transport ferroviaire des marchandises, DORS/91-488, 14 août 1991 si lesdites marchandises sont transportées par chemins de fer; ou
- du connaissement du transporteur maritime, conformément aux dispositions et règlements énoncés dans les tarifs s’appliquant au transport par eau; ou
- du connaissement prescrit par les statuts et règlements se rapportant aux services de camionnage y inclus les stipulations et conditions minimales de transport si lesdites marchandises sont transportées par camion; ou
- du connaissement selon le Règlement sur les exigences applicables aux connaissements, 1198-99 du 3 novembre 1999, si les marchandises sont expédiées de la province de Québec par camion;
et dont l’expéditeur convient et exprime son acceptation pour lui-même et ses ayants droit.
